➜Before applying online, a candidate will be required to have a scanned (digital) image
of his/ her photograph, signature, left hand thumb impression and handwritten
declaration as per specifications given below.
A. The text for the hand-written declaration is as follows:
“I,______(Name of the candidate), Date of Birth ______, hereby declare that all the
information submitted by me in the application form is correct, true and valid. I will
present the supporting documents as and when required. The signature, photograph
and left thumb impression is of mine”.

➜ऑनलाइन आवेदन करने से पहले, उम्मीदवार को अपनी फ़ोटोग्राफ, हस्ताक्षर, बाएँ हाथ के अंगूठे का निशान और हस्तलिखित घोषणा की स्कैन (डिजिटल) छवि नीचे दी गई विनिर्देशों के अनुसार तैयार रखनी होगी।
A. हस्तलिखित घोषणा के लिए पाठ इस प्रकार है:

“मैं, ______ (उम्मीदवार का नाम), जन्म तिथि ______, यह घोषणा करता/करती हूँ कि आवेदन पत्र में मेरे द्वारा दी गई सभी जानकारी सही, सत्य और वैध है। आवश्यक होने पर मैं सहायक दस्तावेज़ प्रस्तुत करूंगा/करूंगी। हस्ताक्षर, फ़ोटोग्राफ और बाएँ हाथ के अंगूठे का निशान मेरा ही है।”
